#### Requirements

- Fully complete and sign the RFQ, including SBD4 and SBD6.1 forms. 
- Additional bid conditions: pass SIU Internal Integrity Unit screening or vetting, comply with OHS Act 37(2), use approved chemicals, maintain safety/environmental records, and deliver per contract terms.
