## Summary

#### Overview
**SAFCOL seeks to appoint a service provider to conduct an occupational hygiene survey on selected SAFCOL offices, workshops and plantations.** Bidders must be a suitably qualified and registered Approved Inspection Authority with a certified occupational hygienist, able to perform fieldwork Sep–Oct 2026, and to submit three quotations (one per survey).

#### Requirements
- Administrative/compliance
- Complete the RFQ on company letterhead; email submissions to the specified address; include SBD forms; provide SARS tax clearance and CSD registration; supply B-BBEE certificate or sworn affidavit; JV bidders must provide consolidated BEE evidence.

- Mandatory qualifications (Phase 2)
- Valid Letter of Good Standing (COIDA)
- Valid Competent Person Certificate for Occupational Hygienist (SAIOH)
- Valid Approved Inspection Authority certificate (Department of Labour)
- Valid SANAS accreditation with Schedule of Accreditation
- Valid calibration certificates

- Project specifics
- Conduct surveys at 26 business units with site visits coordinated with Management
- Use calibrated electronic measuring devices; devices calibrated prior to use
- Reports finalised in Word/Excel with photos; delivered with invoice by 30 November 2026
- Transport and accommodation at bidder’s expense; pre-arranged site visits
- Three quotations must be submitted (one per survey); forestry experience advantageous

- Evaluation and preferences
- 80/20 preference points; 51%+ Black Owned entities earn up to 20 points
- Documentation to prove specific goals (e.g., B-BBEE, ownership, etc.) will be required
