#### Requirements
- CSD registration; current SARS tax clearance; rates & services clearance; BBBEE certificate to claim preference points
- Complete physical examination to be conducted by an occupational health practitioner
- Price per employee; price to include VAT (if registered) and all disbursements
- Coverage: comprehensive medical surveillance for identified employees in Graaff-Reinet, Jansenville, Willowmore
- Tests and services to be provided: medical history, physical exam, spirometry, vision, audiometric screening, keystone screening, height questionnaire, Hepatitis A & B vaccinations, state of fitness
