## Summary

#### Overview
**Provision of Occupational Health Medical Tests services on an as and when required basis for a period of 36 months.** Overberg Water invites professional service providers to supply occupational health medical tests for employees across multiple test sites for 36 months. Suitable bidders include HPCSA-registered occupational health practitioners with MBChB qualifications, experience in occupational health testing (preferably in water/public sector), and capability to conduct tests at three centres.

#### Requirements
- Registered Medical Practitioner (HPCSA) with MBChB; submit proof of registration and qualifications; must be able to perform all listed examinations. 
- COIDA compliance or proof of coverage; include COIDA certificate/letter.
- BBBEE verification: valid SANAS certificate or sworn affidavit (EME/QSE options allowed).
- Central Supplier Database (CSD) registration and Tax Compliance PIN; or CSD printout if not TCS available.
- Minimum 5 years’ experience in overseeing occupational health/medical testing in water sector and/or government; provide CVs and references (including at least one for a water/public entity).
- Proposals must cover all listed tests (baseline/pre-placement, periodic, exit, plus specialised exams and reports) and include travel/accommodation where needed; pricing per examination including VAT.
- Functionality threshold: minimum 70/100 for Phase 3; price and specific goals account for 80/20 (or 80/20/90/10 as applicable).
