## Summary

#### Overview
**Procurement of an Employee Assistance and Wellness Programme (EAWP) for Mkhondo Local Municipality, to be provided as needed over 24 months.** Bidders should be experienced EAP providers with HPCSA-registered directors, and able to supply clinical/counselling psychology services; additional credentials (BBEE, CSD, tax status) strengthen eligibility.

#### Requirements
- Director’s valid HPCSA registration (mandatory).  
- Tax compliance status (TCS/PIN) and SARS e-filing readiness.  
- Full Central Supplier Database (CSD) registration and not older than 30 days.  
- Municipal accounts for bidder and directors, or valid lease if renting (not in arrears >90 days).  
- Certified company registration and IDs of directors; Letter of Authority to sign bid.  
- Proof of HPCSA registration for directors; CVs for key HPCSA-registered personnel (at second stage).  
- Evidence of premises/facilities for consulting/treatment (owned, leased, or LOI).  
- Returnable documents: Schedule 1–4, 6–8; MBD 1–9 and related declarations.  
- Compulsory enterprise questionnaire; Proof of CSD; Company references; Schedule of Tenderer’s Experience.
