#### Requirements
- Valid Undertaker’s Certificate of Competency from the eThekwini Health Unit.
- Premises must pass site inspection (Section 10 Annexure A).
- Registration on the National Treasury Central Supplier Database (CSD) as a service provider (MAAA number required).
- Tax Compliance Status PIN or Tax Clearance Certificate; SARS registration.
- VAT status and Declaration of Municipal Fees (or acknowledged debt).
- Occupational Health and Safety Act declaration and safety plan capability.
- Returnable documents: Authority of Signatory; Official Tender Form; MBD 4, 5, 6.1, 8, 9; Joint Venture agreement if applicable; BBEE documentation.
- If applicable, joint venture documents and manufacturer/distributor agreements.
