#### Requirements
- Accredited service provider; registered as Occupational Hygienist; approved inspection authority with Department of Labour.
- Bidder must submit CK company registration; CSD registration; SARS Tax Compliance Status PIN (or proof); certified IDs for directors (within 3 months validity).
- All requested documents certified within 3 months; comprehensive JV agreement if applicable; complete Form of Offer and declaration of interest.
- Proof of municipal rates/service account not older than 3 months; no tipex; no changes to prices after signing.
- Bids must be valid for 90 days; no bids from state employees; adhere to PPPFA 80/20 (Price 80, BBBEE 10, Specific Goals 10); localities considered.
- Tax, KYC and BBBEE documentation (MBD forms) required; CSD and TB/TAX checks apply.
