## Summary

#### Overview
**Appointment of a service provider to conduct medical screening/occupational health surveillance at Ithala Trade Centre (ITC).** Bidders should be experienced in occupational health screening (physical/mental health questionnaire, audiometry with calibration, spirometry, physical exams, vision, HIV testing) and able to deliver formal medical reporting in South Africa with adherence to PPPFA, GCC, and related contract terms.

#### Requirements
- Registrar on the National Treasury Central Supplier Database (CSD).  
- In good standing with SARS; provide Tax Compliance Status (TCS) PIN or CSD number if no TCS.  
- Complete and sign SBD 1 (Invitation to Bid) with a supporting resolution if not a sole proprietor.  
- Complete and sign SBD 4 (Declaration of Interest).  
- Not be in the service of the State; no disqualifying relationships with Ithala staff or suppliers.  
- In consortia/JV/sub-contracting, each party must submit separate TCS / PIN / CSD where applicable.
