#### Requirements
- Capability to deliver the full medical surveillance package (audiometric, lung function, physical/vision screens, HIV testing, full medical report). Calibration certificates for equipment must be maintained.
- Administrative compliance: 
  - Registration on the National Treasury Central Supplier Database (CSD) and SARS tax compliance status (PIN/TCS). 
  - If a consortium/JV/sub-contractors are involved, each party must submit separate TCS/PIN/CSD. 
  - If no TCS, provide the CSD number; not in the state’s service.

- Mandatory bid documents: SBD 1 (invitation to bid with resolution letter if not sole proprietor) and SBD 4 (declaration of interest).
- Compliance with procurement rules (PPPFA, GCC) and contract via Ithala’s SLA; lowest price not guaranteed.
