#### Requirements
- Provide serviced chemical toilets per Annexure A (5 female cubicles, 2 male cubicles + 3 urinals, 1 accessible male, 1 accessible female).  
- Units with lighting; delivery and full installation/setup; weekly servicing and waste removal.  
- Monthly rental for 24 months; 2-month cancellation notice; Iziko may terminate earlier or extend.  
- Administrative/compliance documents: Central Supplier Database report with supplier details and Tax Status Pin; tax registration number; valid B-BBEE certificate or sworn affidavit; signed RFQ; minimum 5 client reference letters (not older than 5 years).  
- SBD 4 (Bidder Disclosure) and SBD 6.1 (Preference Points Claim Form) completed.  
- Pricing must include VAT and any applicable costs; quotes valid 90 days.
