## Summary

#### Overview
**RFQ for the appointment of a service provider to deliver OHSACT, SHE REP TRAINING, INCIDENT INVESTIGATION and FIRST AID AND FIRE FIGHTER LEVEL 1 for PRASA KZN, for a 12-month period as and when required.** Bidders should be SETA-accredited in the listed areas, able to provide on-site delivery at PRASA premises in Durban, with named presenters and on-site assessments. (See attached document KZN-PRASA-2026-06-06-Q-SAFETY TRAINING.pdf.)

#### Requirements
- Mandatory qualifications and compliance:
- Completion of RFQ, submission by 23 June 2026 12:00, in the PRASA Tender Box No.04 (Durban). 
- Tax compliance: provide SARS PIN or Tax Clearance Certificate; Central Supplier Database (CSD) number; separate certificates for JV/Consortium if applicable.
- Accreditation: proof of SETA accreditation for OHSACT, Health and Safety Rep, Incident Investigation, First Aid, and Fire Fighter Level 1; indicate training presenter(s) and provide presenter facilitator/assessor certifications. (see attached)
- Training specifics: on-site PRASA premises delivery; durations per course (OHSACT 12 hours theory + 28 hours practical; Health and Safety Rep 2 days; Incident Investigation 9 hours theory + 21 hours practical; First Aid 2 days; Fire Fighter Level 1 1 day). 
- Preference points: BBEE minimum Level 2; evidence via B-BBEE certificate/affidavit; 51% Black Owned options available.
- Contingencies: funding approval may affect award; price and compliance responsiveness required. (Refer to sections on funding contingency and GCC.)

#### How to Apply
- Submission: deliver bid response to PRASA Tender Box No.04, 65 Masabalala Yengwa Ave, Durban 4001, by 23 June 2026, 12:00. 
- Documents to submit (as applicable): official SBD forms (SBD1, SBD4, SBD6.1), RFQ completion, Tax Clearance PIN or SARS PIN, CSD number, JV/Consortium agreements if any, SETA accreditation proof, presenter names and certifications, completed Pricing Schedule, and any returnable documents listed in the RFQ. 
- Pre-qualifications: Tax compliance, CSD registration; foreign bidders must provide additional information if applicable. 
- Pricing: complete the attached Pricing Schedule; prices in Rand, fixed and inclusive of taxes; delivery period and escalation basis to be stated.
