## Summary

#### Overview
**RFQ to design, supply and deliver 1,000 custom-shaped metal commemorative badges for the National Museum’s 150th anniversary.** Bidders should be SA-registered service providers with demonstrable experience in designing, manufacturing, supplying and delivering custom metal badges, pins, medals or similar items (RFQ BADGES 2026.pdf).

#### Requirements
- Fully completed and signed SBD 4 form.
- Company profile showing background, mission, structure and proven track record, including photos of similar work (badges/medals/pins).
- Fully completed compulsory price schedule.
- At least three signed reference letters and/or purchase orders for similar work.
- CSD registration with supplier report.
- Active SA company with CIPC registration documents.
- Tax compliant with a valid SARS tax PIN.
- BBBEE certificate or signed BBBEE declaration (optional but may earn preferential points).
- Non-compliance may disqualify the bidder; tax non-compliance has a 7-day grace period if a preferred bidder, else disqualification. Bids valid for at least 60 days.
