#### Overview
**RFQ to supply, deliver and set-up of equipment for the Remembrance Sunday Service.** The City of Johannesburg seeks a service provider capable of supplying event equipment (marquee tents, signage, furniture, safety gear, ceremonial items and related services) for an outdoor remembrance ceremony attended by around 700 people. Suitable bidders are event-equipment suppliers with capabilities in marquees, seating, tables, décor, health and safety compliance, and related logistics. Required certifications include OHSA compliance and BBBEE, with standard SCM and tax clearances.

#### Requirements
- Valid Tax Clearance (SARS) or SARS PIN and VAT registration status; original or certified documents.
- BBBEE documentation (BBBEE certificate or sworn affidavit).
- Registration on the Central Supplier Database (CSD) and City of Johannesburg Supplier Database; record vendor numbers on the RFQ cover page.
- Completed Forms A–J as listed (A: Conditions, B: Quotation/Acceptance, D: Authorization, E: MBD4, F: MBD8, G: MBD9, H: Declaration on Municipal Accounts, I: Occupational Health & Safety, J: Preference Points Form).
- CIDB registration if required for the scope (Construction/Building Maintenance) and relevant attachments if applicable.
- Compliance with Occupational Health and Safety Act (Form I) and related terms.
- Price quotes must be in SA rand, exclusive of VAT; firm prices; all pages initialed; sealed bid envelopes; no price changes after closing.
- Forward exchange-rate cover if prices are exchange-rate affected (where applicable).
