**RFQ for occupational hygiene surveys at uShaka Marine World, including noise, illumination, IAQ, heat/cold stress, vibration, hazardous chemical/biological agents, ergonomic risk assessments and reporting, in support of Approved Inspection Authority surveys. Bidders with occupational hygiene expertise, regulatory knowledge and SA SANAS accreditation should consider bidding.**

#### Requirements
- Be registered on the Central Supplier Database (CSD) and on the eThekwini Supplier Portal.
- Submit a tax clearance or SARS TCS PIN; provide TCS PIN or CSD number; if foreign, complete the pre-award questionnaire and provide necessary documentation.
- Obtain SANAS accreditation if required and comply with Department of Employment and Labour requirements.
- Complete and submit mandatory MBD forms (MBD1, MBD4, MBD6.1, MBD8, MBD9) along with other documents, and submit on company letterhead; provide a valid Letter of Good Standing.
- Provide latest CSD registration report and evidence of good standing; comply with GCC and other contract conditions.
