#### Requirements
- Demonstrated PPE supply/delivery experience with traceable proof (appointment letters and completed orders, GRNs).  
- Qualified personnel with relevant experience; evaluation includes 4+ years/3–4 years/2 years/1 year points.  
- Compliance documents: Central Supplier Database Full Report, SARS Tax PIN or Tax Compliance Status, and a valid Service Fee/Quotation.  
- Preference-driven goals evidenced: Local service provider status (Umkhanyakude or uMfolozi), at least 51% Black ownership, 51% women ownership, 51% youth ownership, 51% ownership by people with disabilities; proof required for each as applicable.  
- Environmental and governance disclosures: environmental record, proof of capacity to comply with environmental rules, and related certifications.  
- Completion of mandatory SBD forms and a pro forma Service Providers Agreement (with all sections/pages initialled and signed).
