#### Requirements
- Original valid Tax Clearance certificate; BBBEE certificate or compliant sworn affidavit.
- Municipal rates and taxes clearance not older than 90 days.
- Completed MBD forms: Declaration of Interest (MBD 4), SCM practices (MBD 8), Independent Proposal Determination (MBD 9), Preference Points (MBD 6.1).
- Proof of CSD registration / Supplier Number; company registration documents with directors’ IDs.
- Pricing on firm basis (Pricing Schedule – MBD 3.1), inclusive of taxes; delivery costs included.
- Compliance: no bids from those in the state’s service, no recent blacklisting; quotes on company letterhead; any JV signed by all parties.
