#### Requirements
- Quotation on company letterhead; inclusive of all taxes; PDFs only (no Word/Excel images) per portal rules.
- Original valid Tax Clearance Certificate.
- BBBEE certificate or sworn affidavit (comply with BBBEE Practice Guide 2018).
- Municipal rates/taxes statement not older than 90 days in arrears.
- Mandatory MBD forms: 4 (Declaration of Interest), 6.1 (PPPFA points), 8 (Declaration of Bid Practices), 9 (Independent Proposal Determination).
- Proof of CSD registration / supplier number.
- Company registration documents with IDs of directors/shareholders.
- No quotes from persons in the service of the state; not blacklisted.
- Datasheet/manufacturer name for all items; failure to provide may disqualify.
- Nine separate depot deliveries (depots listed in doc) and 60-day quotation validity.
- Sample units may be requested.
- Specific Goals: SMME requirement (51%+ black-owned by Black People) with PPPFA scoring.
