#### Requirements
- Register on the Central Supplier Database (CSD) and provide supplier number.
- Original valid Tax Clearance Certificate.
- BBBEE certificate or sworn affidavit (compliant per 2018 guide).
- Municipal rates/taxes statement not older than 90 days in arrears.
- Complete MBD forms: Declaration of Interest (MBD 4), Past SCM practices (MBD 8), Independent Proposal Determination (MBD 9), and MBD 6.1 for preferences.
- Proof of lease agreement or municipal account not older than 3 months; not in arrears beyond 90 days.
- Quotations must be on company letterhead and PDF; no email submissions.
