**Supply and delivery of non-contracted maxilla facial plates, screws, sets on rate-based basis** for Frere Hospital (Eastern Cape Department of Health). Target bidders are medical device suppliers with SAHPRA accreditation, capable of supplying a wide range of maxilla facial implants and related screws/plates, in various sizes, on a rate-based contract.

#### Requirements
- Valid SAHPRA certificate indicating supplier type (sole supplier/manufacturer/distributor/agent).  
- Willingness to supply samples at own cost if requested (non-returnable).  
- Complete and sign the required forms: SBD1, SBD3.1, SBD4, SBD6.1.  
- Latest CSD report attached (or provide CSD number/TCS where applicable).  
- Tax compliance: SARS PIN for verification; may submit a printed TCS certificate; separate TCS for each JV/consortium party. If no TCS, provide CSD number.  
- Not in the state’s service terms; comply with GCC/SCC and price schedule requirements.  
- Returnable Schedule 4 (specifications) and Schedule 5 (pricing) completed.  
- Price evaluation follows PPPFA 80/20 (Price 80, Specific Goals 20); specific goals include HDI, Women, Youth, Disability, Military Veterans, Locality.  
- Documentation proving specific goals (CIPRO ownership, municipal lease or address, updated CSD, etc.).
