## Summary

#### Overview
**CSOS seeks to appoint a service provider to deliver an Employee Wellness and Health Programme across CSOS head office and regional offices for 3 years.** Bidders should have proven experience in employee wellness programmes, on-site clinic services, EAP, HIV/AIDS/workplace health, stress management and related wellness interventions, and must meet SA regulatory and procurement requirements (tax, BBBEE, CSD). The tender is open to qualified health/wellness providers capable of 24/7 confidential support and multilingual service delivery.

#### Requirements
- HPCSA certificate for the Director or project lead (valid as at bid closing).  
- Tax compliance: SARS PIN or printed Tax Compliance Status (TCS); separate TCS/PIN for each consortium member.  
- Central Supplier Database (CSD) registration with full CSD report; or provide CSD number.  
- B-BBEE: valid certificate or sworn affidavit; demonstrate ownership/EME/QSE status as applicable.  
- Complete Standard Bidding Documents: SBD 1, SBD 3.3, SBD 4, SBD 6.1.  
- Disclosure: full shareholding portfolio if bidding as company/consortium.  
- No bid by persons in the state service or entities with such connections.  
- Provide a detailed project lead CV, complete Table A of company experience, and client reference letters (not older than 10 years).
