#### Requirements
- CSD registration compulsory.
- Complete MBTC RFQ forms and attach a valid company quotation on letterhead.
- Price must be VAT inclusive and include delivery; price must be broken down.
- Complete the attached SBD 4 and SBD 6.1 forms.
- Evaluation criteria: 80/20.
- Quotation valid for 90 days; no prepayment.
- Submit via email to scm@mandelabaytheatre.co.za or hand deliver to 68 Cape Road, Gqeberha.
