The MBTC will request samples from the top three ranked providers for evaluation.

#### Requirements
- Compulsory MBTC RFQ forms completed on company letterhead; attach valid quotations.
- Price must be VAT inclusive and include delivery; price must be broken down.
- Quotation must adhere to the closing date/time; quotation valid for 90 days.
- Complete the attached SBD4 and SBD6.1 forms.
- CSD registration is compulsory.
