APPOINTMENT OF A CONTRACTOR FOR THE COMPLETION OF THE OUTSTANDING WORKS AT LOGOBATE CLINIC FOR THE DEPARTMENT OF HEALTH, NORTHERN CAPE PROVINCE. Bidders must hold CIDB Grade 7GB (or higher) and have requisite health-facility refurbishment/construction experience; mandatory site briefing, CIDB, COIDA/FEM/RMA, CIPC, CSD registrations, and SACPCMP registrations for construction management and safety roles.

Requirements

• Fully completed Form of Offer, SBD 1; SBD 4; SBD 6.1 (Preference Points).
• Bill of Quantities fully priced; indicate N/A where applicable.
• JV/Consortium agreement signed by all parties (if applicable).
• Proof of active SACPCMP registrations for Construction Manager (PrCPM/PrCM) and Health & Safety Officer (CHSO/CHSM/CHSA).
• Compulsory Enterprise Questionnaire (each JV/Consortium member).
• Tax Compliance Status (SARS PIN) if recommended for award; SARS verification process.
• Addenda acknowledgment; no prohibited alterations to documents.
• For CIDB/JV specifics, ensure lead partner has Grade 7GB or higher; combined grading meets requirements.
• Proof of professional registrations and references for key personnel.
