Overview

This RFQ seeks to appoint a service provider to conduct a workplace ergonomics and OHS risk assessment for Border Management Authority. The scope covers Head Building (3 floors, ~4200 m2) at 67 Koranna Avenue, Doringkloof, Centurion. Requirements

• Tax compliance: provide SARS PIN or Tax Compliance Status (TCS); can submit a printed TCS or, if using CSD, provide CSD number.
• Pre-qualification: BBBEE status certificate or sworn affidavit (for EMEs/QSEs).
• Forms and governance: complete and submit SBD 1, SBD 3 (Pricing Schedule), SBD 4 (Disclosure), SBD 6.1 (PPF), and SBD 7.1 contract forms; include a board resolution authorising signatory.
• Insurance and risk: Professional Indemnity insurance minimum of R160 million; performance security as required by the SCC.
• Compliance and ethics: adhere to PFMA SCM rules; no fronting; disclose relationships and avoid collusive practices.
• Delivery scope: lead time of 30 days; deliver at the specified Border Management Authority location; prices must be firm and inclusive of taxes.
