Overview

APPOINTMENT OF A SERVICE PROVIDER TO PERFORM MEDICAL SURVEILLANCE FOR PERSONNEL AT ROOODEPLAAT (RQIS). The Department of Water and Sanitation seeks an occupational health service to conduct medical surveillance for 20 Roodeplaat staff (Gauteng) including history, physicals, audiometry, spirometry, blood/urine tests, and to issue fitness certificates and maintain confidential medical files. Qualifications include HPCSA/SANC registration, a lead practitioner who is an Occupational Medical Practitioner or Occupational Health Nurse, and at least 3 years’ relevant surveillance experience; and compliance with POPIA, tax/CSD verification, and on-site capability at Roodeplaat.

Requirements

• Valid HPCSA or SANC registration; lead practitioner must be OMP or OHN; evidence required.
• Minimum 3 years’ experience in pre-employment or occupational medical surveillance.
• Compliance with POPIA and protection of participant medical data.
• Mobility/on-site capability to perform examinations at RQIS, Gauteng (Roodeplaat site).
• Documentation: provide sample medical history questionnaire and Certificate of Fitness template; submit calibration certificates for audiometer and spirometer.
• Pre-qualification documents: CSD printout or SARS PIN; tax compliance; complete required SBD forms (SBD 1, SBD 3.3, SBD 4, SBD 6.1) and SBD 7.
• Optional: Price with detailed cost breakdown per employee (consultation, tests, certificate, travel) as specified in the Pricing Schedule.
