RFQ to appoint a service provider to provide BASIC FIRST AID TRAINING LEVEL 01 for the IASP for Ezemvelo KZN Wildlife. Requirements

• Mandatory pre-qualification requirements (refer to page 27 of the RFQ).
• Tax compliance: provide SARS PIN or Tax Compliance Status (TCS); separate TCS for joint ventures; if no TCS, provide CSD number.
• Central Suppliers Database (CSD) registration and up-to-date details at bid close; disqualification if not registered.
• B-BBEE: provide B-BBEE status verification certificate or sworn affidavit to qualify for preference points.
• Complete and submit all official bid documents on PDF only; use one email (no multiple emails).
• Pricing must be all-inclusive (course materials, travel, accommodation, etc.) and valid for 90 days; delivery to destination as specified.
• Compliance forms: SBD 1, SBD 3.3, SBD 4, SBD 6.1, SBD 7 (contract form) as applicable; authority to sign bid.
