Overview

Appointment of a service provider to provide medical surveillance services for the Department of Water and Sanitation: Eastern Operations, as and when required for a period of 24 months. Bidders should have expertise in occupational health, medical surveillance, and related regulatory requirements (SASOM/SASOHN/HPCSA/SAMSA where applicable).

Requirements

• Qualifications and registrations: Medical Doctor with MBChB and Postgraduate Diploma in Occupational Health OR Occupational Health Nurse; valid medical practitioner registration with HPCSA; SASOM and/or SASOHN memberships; SAMSA approval where required; valid spirometry, audiometry, and vision screening certifications; proof of mobile medical unit.
• Operational capacity: Certificate of practice; evidence of a medical unit within 150 km of Midmar Dam; ability to perform Midmar Dam, Klipfontein Dam, and site entry/exit medicals; calibrated equipment (audiometer, spirometer, vision screener) with calibration certificates; Bill of Quantities completed.
• Administrative/compliance: Registration with CSD (MAAA) and SARS Tax Compliance PIN; active CIPC registration; B-BBEE certificate or sworn affidavit; returnable documents completed (SBD1, SBD3.1, SBD4, SBD6.1); letter authorising signing; GCC compliance.
• Evaluation criteria: 80/20 preference system (price vs. specific goals); minimum three reference letters; mandatory documentation in Phase 1; functionality, administrative compliance, and price/goals in subsequent phases.
