Overview

This tender seeks the appointment of a service provider to render an Employee Wellness Programme for 24 months for Limpopo Economic Development Agency (LEDA). The programme covers LEDA employees (about 280) and their immediate family members, with services including tele-counselling, face-to-face counselling, CISD, health risk screening, monitoring/evaluation, administration, marketing and training. Requirements

• Tax and registration compliance: Tax Compliance Status (TCS) PIN with SARS; if no TCS, provide CSD number; submit unique SARS PIN; ensure compliance or bid may be invalid.
• Registration/certification: proof of SACSSP and HPCSA registrations for affiliates/staff; EAPASA registration for eligible personnel.
• Documentation to submit: company profile; CVs of all personnel; proof of SACSSP/HPCSA; project plan (not just a proposal); reference letters; pricing schedule (SBD 3.3); certified qualifications; evidence of joint ventures (if applicable); pricing details.
• Evaluation framework: bid will undergo Pre-qualification (Phase 1), Technical Evaluation (Phase 2, 100 points), and Price & Specific Goals (Phase 3, 100 points under 80/20 or 90/10 rules); bidders must meet minimum criteria to progress (including 70/100 in Phase 2).
• Specific goals: demonstrate ownership/disadvantaged status for points (Black ownership, Women, Youth, Disability, SMEs, Limpopo location, Military Veterans) with documentation.
