Overview

BEY-SCM-628 invites quotations for the provision of medical surveillance services for municipal employees. Bidders should be qualified occupational health service providers capable of comprehensive medical surveillance (history, physical exam, spirometry, vision and audiometric screening, vaccinations, and fitness assessment) and must be registered on the CSD, hold a SARS tax clearance, rates-and-services clearance, and a BBBEE certificate to claim preference points. Requirements

• CSD registration; current SARS tax clearance; rates & services clearance; BBBEE certificate to claim preference points
• Complete physical examination to be conducted by an occupational health practitioner
• Price per employee; price to include VAT (if registered) and all disbursements
• Coverage: comprehensive medical surveillance for identified employees in Graaff-Reinet, Jansenville, Willowmore
• Tests and services to be provided: medical history, physical exam, spirometry, vision, audiometric screening, keystone screening, height questionnaire, Hepatitis A & B vaccinations, state of fitness
• Pricing schedule (per MBD 3.1) and declaration forms (MBD 4, MBD 6.1, MBD 8, MBD 9) to be completed and submitted
• Council reserve right to accept any tender or part thereof; not necessarily the lowest tender

• Submission deadline: 12:00 on 20 August 2026; tender box in the Robert Sobukwe Building; sealed envelopes clearly marked "BEY SCM 628"; quotations faxed, emailed, or late will not be accepted
