Overview

Procurement of Power Pallet Truck training for 10 personnel to meet driven machinery regulation requirements under the Occupational Health and Safety Act. Training will be delivered at iThemba LABS Cape Town facility for first-time users and operators. Submitters must meet NRF/iThemba LABS processes (CSD registration, BBBEE, tax compliance) and provide certificates on completion. Requirements

• On Central Supplier Database (CSD) registration and provide CSD Summary Report (RFQ .pdf)
• Tax/compliance: tax PIN verification or proof of tax compliance (SBD 6.1) (RFQ .pdf)
• Complete and submit mandatory forms: SBD 4 and SBD 6.1; BBBEE certificate or sworn affidavit for specific goals (RFQ .pdf)
• Provide trainer CV and company profile (RFQ .pdf)
• Training per Appendix 1, Code F12 Driven Machinery Regulations; training at iThemba LABS; trainer to bring training aids/materials (RFQ .pdf)
• Pricing valid for 60 days; firm delivery period; late proposals not accepted (RFQ .pdf)
• Provide pricing schedule including training (10), certificate delivery (10), travel/accommodation if applicable
• Payment terms: 30 days from receipt of invoice; advance payments may require a bank-guarantee (RFQ .pdf)
