Overview

Provision of Employee Wellness and Medical Surveillance Services for a period of three (3) years for Lesedi Local Municipality. Suitable bidders are those with proven employee wellness/occupational health capabilities, able to deliver medical surveillance programs and related support services, and who can meet tax clearance, CSD registration, and B-BBEE/compliance requirements.

Requirements

• Demonstrated experience in employee wellness and occupational health services.
• Tax clearance requirements: SARS Tax Compliance Status (TCS) PIN or valid TCS; where applicable, TCS or PIN for each consortium member; CSD number if no TCS.
• Central Supplier Database (CSD) registration full report and municipal accounts not older than 3 months; valid lease if applicable.
• Completed MBD documents: MBD1, MBD2, MBD4, MBD5, MBD6.1, MBD8, MBD9; Authority for Signatory; Schedule of Quantities.
• Proof of B-BBEE status (certificate or sworn affidavit) and proof of area/origin where required.
• Documents for joint ventures/consortia as applicable (joint venture formation, and separate TCS/CSD for each party).
