Provision of ORHVS (Operating Regulations for High Voltage Systems) Training for Prince Albert Municipality. Requirements

• Completion and submission of the mandatory bid forms (MBD1, MBD2, MBD3.1, MBD4, MBD6.1, MBD7.1, MBD7.2, MBD8, MBD9, MBD15).
• Tax clearance/certification and CSD registration; provide Tax Pin, CSD number, and related status.
• B-BBEE certificate or sworn affidavit; CIDB registration if applicable.
• Central Supplier Database (CSD) and CIDB details for the bidder and any JV/subcontractors; attach relevant documents.
• Attach municipal accounts for directors if required for locality points; ensure signatory authority (board resolutions) and other governance documents.
• Pricing schedule on official letterhead and complete pricing (MBD3.1).
• Compliance with GCC/SCC conditions and local procurement policies (80/20, locality, BBBEE).

• Submission: Place the bid in a sealed envelope marked with RFQ number and title; deliver to the tender box at the Thusong Centre, Adderley Street, Prince Albert, 6930, or post to Private Bag X53, Prince Albert, 6930. Late, faxed, or e-mailed submissions are not accepted.
