Overview

Procurement of Medical Surveillance Services for Cederberg Municipality. • Bid documentation: Complete and sign all pages; provide MBD 1, MBD 4, MBD 6.1 (B-BBEE), MBD 7.1, MBD 8, MBD 9; Declaration of Interest; Certificate of Independent Proposal Determination.
• Financial: Price validity at least 60 days; invoices payable within 30 days; prices VAT inclusive.
• Special conditions: Intramuscular vaccine administration; supply MSDS with bid; 3-dose Hep B/A&B schedule within 6 months; no work beyond defined scope.
• Compliance: No bids from entities in the State’s service; adhere to GCC/SCC; provide local and B-BBEE preference documentation.
