Overview

Appointment of a service provider for medical screening to be conducted at Ezakheni, Industrial Estate. Open tender for providers with occupational health screening capabilities, including physical/mental health questionnaires, audiometric screening, lung function, physical examinations, vision, voluntary HIV testing, and occupational medical reporting; and who can meet the procurement’s administrative and tax/compliance requirements (CSD/SARS PIN).

Requirements

• Must be registered on the National Treasury Central Supplier Database (CSD) and be in good standing with SARS; provide CSD or SARS PIN as applicable; separate CSD/TCS for consortia/joint ventures.
• If no TCS PIN is available, provide CSD number; ensure tax compliance per SARS.
• In consortia/joint ventures, each party must submit separate TCS/PIN/CSD.
• Provide SBD 1 (with resolution letter if not a sole proprietor) and SBD 4 (Declaration of Interest).
• Ensure compliance with bidding requirements and complete all pages; late or non-conforming bids are eliminated.
• Evaluation uses 80/20 (price vs. specific goals) or 90/10 rules per PPPFA regulations; specific goals include: ≥51% Black ownership, ≥51% Women ownership, ≥51% Youth ownership, ≥25% People living with disabilities, with certified proof required (certified copies by commissioner of oaths).
• The contract, if awarded, will be via Ithala Service Level Agreement; standard terms apply and are not negotiable.
• Anti-fraud/ethics: avoid conflicts of interest; anti-corruption provisions and reporting mechanisms are specified.
