Overview

Appointment of a service provider to conduct medical screening (medical surveillance) at Madadeni Industrial Estate. The contract targets on-site occupational health screening services (audiometry, lung function, physicals, vision, HIV testing, and reporting) for 11 employees. Suitable bidders are occupational health providers with capability to perform the listed screenings, generate full medical reports, and operate at a client site.

Requirements

• Bidder must be registered on the National Treasury Central Supplier Database (CSD).
• Bidders must be in good standing with SARS; provide Tax Compliance Status (PIN) or CSD number if no PIN.
• Complete and submit official bid forms: SBD 1 (Invitation to bid) with resolution, SBD 4 (Declaration of Interest); SBD 6.1 (Preference points form) as applicable.
• For consortia/joint ventures, each party must submit separate TCS/PIN/CSD.
• No bids from personnel in the state or entities with state connections; comply with tax, ethics, and SCM rules.
• Include pricing on the provided Pricing Schedule; VAT details (15%) apply as indicated.
• Preference points available for specific goals (ownership by HDI categories) with documentary proof (e.g., CIPC certificates for ≥51% Black/Women/Youth ownership; disability proof).
