Requirements

• Complete and submit the SBD4 Bidder’s Disclosure (forms included in the attached SBD4 documents).
• Disclose any tender defaulter lists, relationships with the procuring entity, and any CSD-registered interests (failure to disclose linked active CSD entries may lead to disqualification).
• Complete Annexure B Sworn Affidavit for EME/QSE (points allocated only if returned with the RFQ).
• If applicable, provide addenda sworn affidavits as per the Addendum 4 template.
