Overview

This RFQ seeks to appoint a service provider to carry out renovations to SS GIDA HOSPITAL, AMATHOLE DISTRICT (windows and doors repairs) under a once-off 3-month contract. Suitable bidders are CIDB 2GB+ in the 2GB class of works; procurement follows a JBCC Minor Works Edition 5.2 process with a two-stage evaluation. Requirements

• CIDB: minimum 2GB grading (class of works) and active CRS number
• Valid Department of Labour COIDA or FEM Good Standing
• VAT registration (if VAT vendor) and indicate VAT; otherwise include VAT in bids
• Registration on the National Treasury Central Supplier Database (CSD)
• Returnable documents: SBD 1, SBD 4, SBD 6.1; Compulsory Enterprise Questionnaire; 3 references with Completion Certificates; Part C1.1 Form of Offer and Acceptance; Part C1.2 Contract Data; Part C2.2 Bills of Quantities (handpriced); POPIA consent; Record of Addenda
• Proof of CIDB certificate and CRS number
• Specific goals documentation and any required amendments/qualifications
• Tender validity: 90 days; price to be fixed for contract duration
• Retention: 10% security on award
