Overview

Provision of Safety, Health and Environmental (SHE) Representative Training for 40 delegates (split into 4 groups) to be delivered at an eThekwini Municipality venue, with QCTO SP-250815 accreditation. Bidders must be registered on the Central Supplier Database (CSD) and on the eThekwini Supplier Portal and provide a SARS tax clearance or PIN.

Requirements

• CSD registration and eThekwini Supplier Portal registration
• Tax clearance status or SARS PIN (TCS) and proof; separate TCS/PIN for consortium situations
• Proof of training provider accreditation: QCTO SP-250815
• Complete and sign MBD forms (MBD1, MBD4, MBD6.1, MBD8, MBD9) on company letterhead
• Stage One admin compliance checks (latest CSD report; SARS good standing; etc.)
• Optional: Consolidated BBBEE certificate for JV/Consortium
• Venue within eThekwini, compliant with OHS Act; inspected; emergency plans; accessible facilities
• Learning resources and equipment aligned with programme; attendance certificates and QCTO certificates
