Overview

Procurement of medical waste management services for Johannesburg Water’s JW Clinic, including supply of biohazardous waste boxes and sharps containers, on-site weighing, and regular collection/disposal. Bidders should have expertise in medical/hazardous waste management, relevant permits/licenses, and be able to operate within Johannesburg, with BBBEE and CSD registration.

Requirements

• Original valid Tax Clearance certificate; BBBEE certificate or compliant sworn affidavit.
• Municipal rates and taxes clearance not older than 90 days.
• Completed MBD forms: Declaration of Interest (MBD 4), SCM practices (MBD 8), Independent Proposal Determination (MBD 9), Preference Points (MBD 6.1).
• Proof of CSD registration / Supplier Number; company registration documents with directors’ IDs.
• Pricing on firm basis (Pricing Schedule – MBD 3.1), inclusive of taxes; delivery costs included.
• Compliance: no bids from those in the state’s service, no recent blacklisting; quotes on company letterhead; any JV signed by all parties.

Mandatory requirements also include providing copies of legal authorizations/permits and a disposal certificate with signatures (after disposal).
