Requirements

• Must be registered on the Central Suppliers Database (CSD) and provide the CSD PIN and report.

• B-BBEE: submit B-BBEE status certificate (MBD6.1) or sworn affidavit.
• Complete Listing Criteria (Mandatory in BID documents) and declare conflicts of interest (MBD4).
• Submit all mandated MBD forms: MBD1, MBD2, MBD3.1, MBD4, MBD6, MBD8, MBD9, plus GCC/SC as applicable.
• If a joint venture, provide JV agreement and related documents.
• Pricing: complete Pricing Schedule (MBD3.1) with minimum upset price; no correction fluids.
• Ensure all pages are signed and witnessed where required.
