Requirements

• Quotes on PDF, on company letterhead; brand names as required
• Submit a signed Joint Venture Agreement if applicable
• BBBEE certificate or sworn affidavit
• Lease agreement or municipal account not older than 3 months and not in arrears beyond 90 days
• Proof of CSD registration/supplier number
• Complete and submit MBD forms (e.g., MBD 4, MBD 8, MBD 9, MBD 6.1) as applicable
• Pricing to be firm and inclusive of all taxes
