Procurement of PPE (once-off) for Kannaland Municipality (FQ 18/2026). Requirements

• B-BBEE certificate or sworn affidavit; BBBEE status level of contributor.
• Signed MBD10-family forms: MBD1, MBD3.1 (pricing), MBD4 (declaration of interest), MBD5, MBD6.1, MBD6.2, MBD8, MBD9; MBD7 contract form.
• CIDB registration (if applicable) and proof of municipal rates/accounts for bidder/directors.
• Authorized signatory: board resolution authorizing signatory (or JV lead), with sample certificate of authority.
• Data sheets for all products; proof of distribution appointment if not manufacturer.
• Firm prices (pricing schedule) and delivery costs included; pricing schedule in Pricing Schedule - PPE for Community.xlsx.
• Site briefing not applicable; samples may be requested; delivery terms include advance notice and delivery to Ladismith Stores.
• Validity of bids: 90 days from closing date.
