Overview

Supply and delivery of medical surveillance for 60 contract workers in Gauteng and the Eastern Cape. Bidders should be occupational health service providers capable of delivering comprehensive medical surveillance (physical examinations, spirometry, audiometry, vision screening, fluoride testing, blood tests for pesticides/herbicides, skin examinations, and monitoring for water-borne diseases) and able to operate across the two provinces. Compliance and certification requirements include tax/compliance and BBEE considerations typical for public sector work.

Requirements

• Demonstrated capability to perform full medical surveillance for 60 workers across specified tests and screenings.
• Tax compliance: provide SARS PIN for verification or a Tax Clearance; if no TCS is available, provide a CSD number; consortia must submit separate TCS for each party.
• Documentation: submit SBD forms (SBD1, SBD3.1, SBD4, SBD6.2) signed; acceptance of GCC/SCC; SBD7 where required.
• Registration/pre-qualification: tax clearance certificate from SARS; CSD number if no TCS; VAT registration; supplier registration with DWS (if not already registered); B-BBEE documentation (certificate or sworn affidavit) and HDI preference forms where applicable; joint ventures must attach JV agreements.
• Restrictions: ensure no disqualifying links to state employment; disclose sub-contracting arrangements and HDI status of sub-contractors if any.
