Overview

Supply and delivery of non-contracted maxilla facial plates, screws, sets on rate-based basis for Frere Hospital (Eastern Cape Department of Health). Target bidders are medical device suppliers with SAHPRA accreditation, capable of supplying a wide range of maxilla facial implants and related screws/plates, in various sizes, on a rate-based contract.

Requirements

• Valid SAHPRA certificate indicating supplier type (sole supplier/manufacturer/distributor/agent).
• Willingness to supply samples at own cost if requested (non-returnable).
• Complete and sign the required forms: SBD1, SBD3.1, SBD4, SBD6.1.
• Latest CSD report attached (or provide CSD number/TCS where applicable).
• Tax compliance: SARS PIN for verification; may submit a printed TCS certificate; separate TCS for each JV/consortium party. If no TCS, provide CSD number.
• Not in the state’s service terms; comply with GCC/SCC and price schedule requirements.
• Returnable Schedule 4 (specifications) and Schedule 5 (pricing) completed.
• Price evaluation follows PPPFA 80/20 (Price 80, Specific Goals 20); specific goals include HDI, Women, Youth, Disability, Military Veterans, Locality.
• Documentation proving specific goals (CIPRO ownership, municipal lease or address, updated CSD, etc.).

How to Apply

• Submission deadline: 23 June 2026 at 11:00. Submit on CSD or the e-Tender Portal; email/courier submissions are not accepted. (see RFQ MAXILLO_Plates_Etal.pdf)
• Submit using official documents: SBD1, SBD3.1, SBD4, SBD6.1, pricing schedules, and tax/compliance certificates.
• Provide a current SARS PIN/TCS as required; if in a JV/consortium, each party must provide separate TCS/PIN.
• Include the latest CSD report; ensure Total Price on CSD matches RFQ document.
• Access bid documents via the RFQ (RFQ_MAXILLO_Plates_Etal.pdf); briefing on-site address is Connaught/Amalinda Main Road, Amalinda, East London 5100.
• Evaluation follows: Stage 1 administrative compliance; Stage 2 non-negotiables; Stage 3 price and goals; Stage 4 samples (if required).

Notes: The department reserves rights to accept/reject bids and to withdraw at any time. The purchase involves multiple rate-based providers for Maxilla Facial Surgical Implants.
