THE SUPPLY, DELIVERY, INSTALLATION, COMMISSIONING AND MAINTENANCE OF NUCLEAR MEDICINE EQUIPMENT AT VARIOUS GAUTENG HEALTH INSTITUTIONS FOR A PERIOD OF THREE YEARS

Overview

The Gauteng Department of Health invites bids for the supply, delivery, installation, commissioning and maintenance of Nuclear Medicine equipment at various Gauteng Health Institutions for a period of three years. Bidders should be experienced medical equipment suppliers with SAHPRA licensing and capabilities for installation, commissioning, maintenance, training, and warranty coverage, and must meet relevant quality and regulatory standards (SAHPRA, IEC 60601/60601-1-2, ISO 13485).

Requirements

• SAHPRA certificate authorising manufacture/import/distribution of medical devices; compliance verifications may be performed (Section 1; Annexure A).
• If not the manufacturer, a valid commitment letter from the original manufacturer or reseller; or a letter confirming own production/distribution if manufacturer (Section 1).
• Attendance at the compulsory briefing session; submission of a stamped briefing certificate; integrity pact with GPG (Section 1).
• Tax compliance: SARS PIN (TCS) or CSD number; separate TCS for joint ventures/consortia; proof may be required (Section 1, 2).
• CSD registration (MAAA), proof of ownership if signing SBD forms as owner, and GPG Integrity Pact (Section 1).
• Complete and submit SBD 1, SBD 4, SBD 6.1; pricing in SBD 3.2 and Annexure B; product brochures/technical data; Annexure A compliance (Section 1 and Section 2).
